Cuts occur in Lethwei, and they need medical attention. The referee can stop a fight and refer a heavily wounded fighter to the doctor.

Lethwei is fought without gloves. Fighters may wear only tape, gauze and electrical tape on their hands and feet.

Concussion is a risk wherever the head can be struck, and Lethwei permits punches, elbows, knees, kicks and headbutts.

A Lethwei bout can be booked as three, four or five rounds of three minutes, with a two-minute break between rounds. Under the traditional rules, a fighter who is knocked out or injured can take one special two-minute timeout per fight and then choose whether to continue. The 1996 tournament ruleset removed the timeout, and the WLC adopted that ruleset.

None of this means Lethwei is safe, and no combat sport is. We have not found published injury or concussion data for Lethwei, so this page makes no claim about how often any injury occurs or how the sport's risks compare with those of any other combat sport.

The dangers of the headbutt are real. Cuts occur, and they need medical attention.

Lethwei is legal in Myanmar. In 1996 the Myanmar Traditional Lethwei Federation (MTLF) created the tournament ruleset for the inaugural Golden Belt Championship tournament.

One referee oversees a Lethwei bout. The referee can stop the fight and refer a heavily wounded fighter to the doctor. A technical knockout is when a fighter forfeits, has an injury, or is in a position that can damage or severely harm him if the fight continues; the ring doctor is consulted and makes the decision.

We have not found a published medical policy from either the WLC or the Myanmar Traditional Lethwei Federation. If either body has one, it has not, as far as we can verify, been made public. Traditional sandpit fights in rural Myanmar do not always have ringside physicians.

“အရမ်း ရက်စက်ကြမ်းကြုတ်တယ်”

Yes, Lethwei is a full-contact combat sport fought without gloves. Yes, headbutts are legal. Yes, there is blood. Carvings on temples and pagodas in Bagan appear to show pairs of men locked in combat, suggesting the sport is potentially over a thousand years old.

“ကလေးများကို လေ့ကျင့်ပေးတယ်”

Children do fight in Lethwei: in rural areas, having a skilled child fighter has been a way of escaping poverty. We have not found a published source on the rules or medical oversight that apply to child fighters, so this page makes no claim about them.

“ဘယ်တော့မှ လူသိများမှာ မဟုတ်ဘူး”

The World Lethwei Championship was available in over 100 countries through broadcast deals, and WLC 7 was the first Lethwei event broadcast internationally live on UFC Fight Pass. The WLC held no events after 25 September 2020.

The informed consent argument is the usual foundation of the ethical case for combat sports: adults who understand the risks and choose freely to compete under established rules are exercising bodily autonomy. The argument holds only to the extent that the fighter is an adult who has actually been informed of the risks.

The cultural heritage argument is that Lethwei is part of a living Burmese tradition. Lethwei went through many years of suppression during British colonial rule and was revived under General Ne Win's government.

The tension between tradition and modern safety standards is real, and it would be dishonest to pretend otherwise. In ancient times, matches took place in sand pits instead of rings, and boxers fought without protective equipment. Evolution is not abolition.

Ultimately, the ethical case for Lethwei rests on a simple proposition: that adults who understand the risks, who choose freely, and who compete under established rules have the right to test themselves.
